Veterinarian Care Assistance Form

LAPS covers non-routine veterinarian costs for your companion animal, with a lifetime maximum of $1000 per animal.  You must income qualify at 100% or less of AMI and live / work in Summit County. (see below for all required documents needed to determine eligibility)

LAPS does not cover routine medical visits defined as but not limited to: wellness exams, vaccinations, food, laser, routine blood tests, microchipping, massage, dental cleaning, medicines or elective procedures. 

LAPS covers 50% of the cost of a spay/neuter surgery for those who income qualify (80% of AMI) and live or work in Summit County.   You must use a local veterinarian for this surgery.

For fastest processing, email completed application and supporting documentation to MedicalApps@summitlaps.org  Applications and supporting documents can also be dropped off at the Summit County Animal Shelter in Frisco to be scanned and sent to LAPS.

To assist in our ability to review your application and provide a timely response, please read the instructions and include ALL requested information with your application.  Incomplete applications will delay the review process.
