Veterinarian Care Assistance Form

LAPS covers non-routine veterinarian costs for your companion animal, with a lifetime maximum of $1000 per animal.

LAPS does not cover routine medical visits defined as but not limited to: wellness exams, vaccinations, food, laser, routine blood tests, microchipping, massage, dental cleaning, medicines or elective procedures.

Eligibility

  • APPLICANTS MUST LIVE OR WORK IN SUMMIT COUNTY COLORADO

  • SINGLE APPLICANTS EARN LESS THAN $55,000 GROSS/YEAR

  • MARRIED/PARTNERS HOUSEHOLD INCOME IS LESS THAN $75,000 GROSS/YEAR

      • (Gross Income includes all sources of income: business, unemployment, disability, etc. before taxes or expenses.)

Required Documents

  1. Completed LAPS Financial Assistance Application.

  2. Written estimate or all invoices from your veterinarian with itemized details of treatment

  3. Proof of Income – provide one for all wage earners (BLACK OUT your Social Security number)

      • 1040 Federal Individual Income tax return (1st two pages only)

      • Unemployment – provide recent check

      • Disability – provide proof of status

      • Medicaid – provide proof of status

  4. Proof of residency or employment in Summit County. (Acceptable proof includes: residence lease, utility/phone bill or a recent paycheck stub that lists your name & current physical address)
